To apply for the 2023 Turning The Tide Ovarian Cancer Retreat click on the links for the following documents, print them out, fully complete and sign the documents, and either mail or email them to Margaret Mastrangelo. Instructions to email/mail the forms are included in the Instructions document.
If you are unable to print these documents you can have them mailed to you by contacting Margaret Mastrangelo at mastrangelom@comcast.net.
You will be notified by phone or email with confirmation of your spot no later than May 30, 2023.
